1.1. The undersigned hereby approves of this application and in doing so agree that the Inglewood United FC and its officers and servants are free and clear of all responsibility whatsoever in the above-mentioned activity for any loss injury or illness during the applicant’s participation in this activity.

2. Cancellation and Refund Policy
2.1. Please make sure all relevant information regarding registration has been carefully read, and fully understood, before signing below, and that conditions are accepted. Upon registration the full amount is payable and, once fees have been paid, there will be no refund.
2.2. De-registration will need to be in writing and addressed to the Club through the Club Registrar
2.3. The Control, Conduct, Administration and Policy on all activities relating to a registered Inglewood United FC player is always, at the
absolute discretion of the Club’s Board of Management.
2.4. It is agreed that no claim or proceeding will be brought against any Football Administrator at Inglewood United FC for any damage, loss,
injury, or liability you may suffer while participating in football, training, or playing in a Match. Except for liability that by law cannot be
excluded, the Football Administrators exclude all liability to you in tort (including negligence), contract or bailment for acts or omissions of
the Football Administrators and their employees, agents, and contractors.

3. Registration Fees & Payment/s
3.1. The registration fees assist in funding of the Club and facilitating annual administrative and operational costs, including (but not limited to):

• Purchase of playing strips (home and away), socks & shorts
• Purchase of training and match day equipment
• Ground hire fees for out-of-season and pre-season training from the City of Stirling
• Ground maintenance fees
• Administrative costs incurred as a club member of the National Premier Leagues WA
• Football West affiliation and team fees
• Football Federation Australia annual levy
• Match day Referee fees
• Engagement of a Club Technical Director with a minimum ‘B’ Licence coaching accreditation
• End of season Trophy presentation event and Trophies
• Facility insurance and maintenance fees
• Club building levy
• Club audit fees
• Complimentary Family Pass to all Inglewood United National Premier Leagues games played at the IUFC Grounds

Accepted forms of payment are via EFT, Credit Card and Cash

4. Play Football

4.1. All Players are required to register on PlayFootball prior to the commencement of the new season. Registrations will be accepted only when club registration fees have been paid in full. No player can play in the new season unless they have registered on PlayFootball.

4.2. All volunteers, coaches and committee members are also required to register on PlayFootball. Follow this link to open the MFC page for

5. Code of Conduct (Players and Members)

5.1. I will abide by the following rules:

• Be a role model, always giving my best, playing without fear of making mistakes and playing for fun and enjoyment
• Never argue with an official. If I disagree, I will request my team captain or coach approach the match official during a break in play or after the match is concluded.
• Respect the officials and spectators
• Control my temper. I understand that verbal abuse of officials, or deliberately distracting or provoking another person is not acceptable or permitted behaviour in sport
• I understand any send-off offence during competition games will be subject to disciplinary action by the Governing Body and/or the Club, which may result in a fine or bond imposed.
• understand as a player I may be liable for payment imposed by the Competition Administrator, unless there are special circumstances to be determined by the Club (only applicable to competitive age competitions)
• Work equally hard for myself and my team
• Be a good sport and applaud all good plays whether they are made by my team or the opposition
• Treat all players as I wish to be treated. I will not bully or take unfair advantage of another competitor
• Cooperate with my coach, team members and opponents. Without them there would be no competition
• Concentrate on the game plan my coach has asked me to play
• Display modesty in victory and graciousness in defeat
• Respect the rights, dignity and worth of all people involved in the game, regardless of their gender, ability, cultural background
• Practice and try to improve
• Remove all jewellery prior to training and match play, as it is a hazard to me and those around me
• Not accept or use any banned or unauthorised drug(s), including the consumption of alcohol at any time
• Arrive and depart the ground as per the “published dress code”
• Behave appropriately when I am representing the club

6. Marketing & Public Relations

6.1. We hereby agree that any photos taken can be used by IUFC for club promotional activities and/or materials

6.2. Your personal data will be used principally to keep you updated on Club related matters. Where the need arises, your data may also be

used, retained and/or shared with other Club representatives, for the following purposes: accounting, billing, customer relations, and

advertising opportunities and to help us in any future dealings with you.

6.3 By providing my email address, I give permission for IUFC to use this email address as a source to communicate Club related correspondence to me.

6.4 Any child entering the competition with a medical condition must bring a letter from his/her doctor regarding treatment of the

condition.

6.5 In the event of any injury or illness to my child, I hereby give consent in arranging any medical treatment that may be required.
